    POLICY: Drug Testing

    PERTAINS TO: All Employees

     

    [Company Name] provides a safe workplace for all employees, and in doing so, tests all employees for controlled substances. Current employees must undergo testing every [insert time span].

    [Company Name] ensures the confidentiality of all results. Only those individuals who need to know the results will have access. In compliance with state law, the results will be sent to the appropriate federal agency. The results will not be used against the individual in any criminal prosecution.

    Employees will be given [insert time span] notice before the test will be given. Employees will also be required to sign a consent form. For prospective employees, employment is contingent upon the results.

    >[Company Name] will cover the financial costs of any drug test it requires. If an employee/prospect requests a retest, [Company Name] will also cover this cost. However, any further retests will be paid by the employee/prospect.

    Only state-licensed laboratories will conduct any and all drug tests required by [Company Name].

    The employee/prospective employee will be notified of results by [insert method] by [insert person].

    If the employee/prospective employee receives notice that the test results were positive, s/he will have the opportunity to explain the test results and/or to request a retest of the same sample.

    DISCIPLINARY ACTION

    If the appropriate company authorities determine that the positive test results were not erroneous, the employee will be terminated immediately and the prospective employee will not be offered employment. In addition, the appropriate government agencies will be contacted
