Force employee to get a check up?

We have a long time employee (16 years) that is somewhat handicapped ( mental more than physical ) who cleans the warhouse and parking lot daily. The past few months it appears his health is really beginning to slide and we are concerned that he will become a liability. He has been seen falling asleep during lunch breaks while sitting up at the table, heavy breathing, serious weight gain and just generally not stable.
He has been with the company since day one and we feel awful thinking about letting him go but at the same time are very concerned he will hurt himself seriously or have a very serious workers comp claim soon.
Can we tell him he is not able to work until he gets a doctor release?
